Located near several Memorial Hermann hospitals, the Memorial Hermann Community Resource Centers were created to help improve the overall health of our community by providing one-stop locations where individuals and families in our community can get help signing up for health care and social services. Staffed by community health workers, the Centers are here to help all members of our community improve their overall health and wellbeing, which can lead to improved quality of life. We discuss ways that people can help meet their needs by providing information on how to access health care, health education and social services.
Knowledgeable about community resources, our team can assist with health and social service connections, applications and education. We help people apply for medical insurance and can connect individuals and families to community clinics. We can also help with access to food through SNAP applications, referrals to community pantries and provision of emergency onsite pantries. Additionally, we help with social service connections, utility, legal, financial and behavioral health and other related needs. Memorial Hermann Community Resource Centers are open Monday through Friday, from 9 a.m. to 5 p.m.
All community members and patients of Memorial Hermann are eligible to receive services from the CRC.
We can assist with Harris Health Financial Assistance Program, state benefit programs such as Medicaid, SNAP, CHIP and Medicare Savings Plan. We also have a food pantry on-site and can provide linkage to medical care and social services.

Most eligibility applications will require proof of income, address and a photo ID, but please call for more information.
