By helping to provide community-based care and resources to improve health outcomes and prevent health issues before they arise, Memorial Hermann Community Health develops programs and services that align with the Greater Houston community’s health priorities. Going beyond improving access to health care, these priorities include the prevention and management of chronic conditions, improving maternal and infant health, and providing mental health services and more. Additionally, our efforts focus on improving the conditions in which people are born, grow, work and live, including expanding access to healthy food and economic and educational opportunities.
