Dr. Carlos Guerra, Jr., Psychiatrist, Mental Health Crisis Clinics - Carlos Guerra, Jr., M.D. received his Bachelor of Science degree in Biology from Texas A & M University in 1985. He received his Doctorate in Medicine from The University of Texas Medical School at Houston in 1989. He completed his residency in Adult Psychiatry from Baylor College of Medicine in 1993 and his fellowship in Child Psychiatry from Baylor College of Medicine in 1994. He is board certified. Dr. Guerra was the Medical Director for the Children’s and Adolescent Unit at Spring Shadows Glen from 1995 to 1999.

While Medical Director for the Children’s and Adolescent Unit, Dr. Guerra was appointment Assistant Clinical Professor of Psychiatry, Division of Child Psychiatry for Baylor College of Medicine. He was the Medical Director for Spring Shadows Glen from 1999 to 2000. He also served as Chief of Staff at Spring Shadows Glen in 1999. He has served as the Medical Director for the Memorial-Hermann Rapid Psychiatric Response Team since 2000. He has been on the clinical staff at PaRC since its inception in November 2000. In August 2008, Dr. Guerra accepted the position as Medical Director for Adolescent Psychiatric Services at PaRC and works with chemically dependent adolescents and adults. He was in private practice for over 20-years and was involved in numerous clinical pharmaceutical research trials. He supervises medical students from UTMB, physician assistant students from University of North Texas Health Science Center, and nurse practitioner students from several different nursing programs.

Since 2011, Dr. Guerra has served as one of two medical directors for Interface EAP. In 2014, Dr. Guerra accepted the position as Medical Director for Memorial Hermann Psychiatric Response Team Case Management. In late 2014, Dr. Guerra accepted the position to oversee Memorial Hermann’s Mental Health Crisis Clinics - Urgent/Emergency Outpatient Psychiatric Clinic. Dr. Guerra is bilingual and is fluent in English and Spanish. He is actively involved with his church.

Dr. Philip Farley, Psychiatrist, Mental Health Clinics - Dr. Farley has over twenty years of experience practicing Child, Adolescent, and Adult Psychiatry. He is a Board Certified Specialist and he has been associated with the Crisis Clinics since 2014.

Dr. Farley currently conducts psychiatric assessments and brief treatments for patients at the Memorial Hermann Crisis Clinic. He is a member of the American Psychiatric Association and the American Academy of Child and Adolescent Psychiatry.

Dr. Ashok Khushalani, Psychiatrist, Mental Health Crisis Clinic – Dr. Khushalani is a psychiatrist who has been in private practice for the past 37 years. He is board certified in child and adult psychiatry as well as addictions. He has worked in academics and in private hospitals and residential treatment centers .He has served as a medical director of several psychiatric units in the past. He has also worked with Tri-county MHMRA and is a consultant to social security administration.

Dr. Khushalani has held clinical appointments at the University Texas. He is currently in private practice and also working at the Memorial Hermann Mental Health Crisis Clinic. He is also a certified Heartmath trainer-specializing in Resiliency Training.

Dr. Khushalani is a distinguished Life Fellow for the American Psychiatric Association and a distinguished Fellow for the American Academy of Child & Adolescent psychiatry. His interests include consultation to the media. He is on the Speakers Bureau for the Physicians Health and Rehabilitation Committee of Texas Medical Association. He serves on the Physician Health and Rehabilitation committee of the Harris County Medical Society. He is also the President of a nonprofit foundation-Khushalani Foundation, which supports various charitable foundations.

Amanda Fischer, PA-C – is a certified physician assistant who has specialized in psychiatry for the past 15 years. She was in private practice for 10 years with Richard B. Pesikoff and Associates and has been with the Mental Health Crisis Clinics for the past 5 years. She is experienced in all facets of mental health including Schizophrenia, Schizoaffective Disorder, Bipolar disorder, Major Depression, Generalized Anxiety, Oppositional Defiant Disorder in and Attention Deficit Hyperactivity Disorder in children. She is able to manage psychiatric emergencies and facilitate hospitalizations when necessary.

Amanda graduated from the University of Houston with a degree in Biology and from The University of Houston with a degree in Physician Assistant Studies. Besides psychiatry, she has also practiced in Neurology and Pain Management.

Michael Chen, MSN, RN, PMHNP-BC - received his undergraduate and graduate’s degrees with honors from the University of Texas Health Science Center Houston. As a Registered Nurse, he worked as a psychiatric charge nurse at Cypress Creek Hospital in a Psychiatric Intensive Care unit as well as house supervisor. Prior to joining Memorial Hermann’s Mental Health Crisis Clinic he worked in private practice, outpatient chemical dependency programs, and inpatient psychiatric hospitals as a psychiatric mental health nurse practitioner.
Michael is ANCC Board Certified as a Psychiatric Mental Health Nurse Practitioner and a member of Psychiatric Advanced Practice Nurses of Houston.

Sheila P Huynh, PA-C – is a certified Physician Assistant who received her training from the University of Dayton. She is experienced in all facets of mental health including child and adult psychosis, mood disorders, and behavioral problems. She is able to manage psychiatric emergencies and facilitate hospitalizations when necessary.
