User Generated Content Consent

By responding to our request with an affirmative answer or the hashtag #MHConsent you are authorizing Memorial Hermann Health System to create, obtain, record, use and disclose photography and/or video recordings across its branded social media channels and website. You acknowledge the images and media to be used by Memorial Hermann Health System were previously posted in a public forum. You further consent to your information, images and other media being stored and managed within Memorial Hermann Health System for future use, unless you indicate otherwise. The permitted uses and disclosures of this information, images and other media may include without limitation:

Posting on Memorial Hermann Health System’s websites (internal and external) or use of social media (Facebook, Twitter, Instagram, LinkedIn, YouTube, Pinterest, etc.) You hereby waive the right to receive a copy, inspect or approve the images and other media, and also waive any and all rights that you may have to any claims for payment or royalties in connection with the above use of the images and other media. You acknowledge that the images and other media will remain the sole property of Memorial Hermann Health System. You also understand that Memorial Hermann Health System is not receiving any financial or other compensation from third parties for use of the images or other media.

By offering consent, you promise the following:

• that you are 18 years or older;
• that if children are depicted in your content, you are the parent/guardian with legal responsibility of any minors depicted
• that you own or otherwise control the rights to your content and any and all elements thereof;
• that you have the rights from any and all third parties appearing in such content to use their names, images or likenesses and any other third party-owned elements as necessary in and as part of your content; and
• that your content does not infringe or violate the rights of any third parties, including, but not limited to, intellectual property rights, copyrights, trademarks, rights of publicity/privacy, patent, trade secret or confidentiality obligations.

You have the right to refuse to use the hashtag #MHConsent for this Consent and that this Consent is valid unless you cancel or revoke it in writing. If you choose to revoke this Consent at any time in the future, you will send my revocation to digitalmedia@memorialhermann.org. This Consent is valid unless revoked in writing.
