The Memorial Hermann Accountable Care Organization (ACO) provides targeted health management services and care coordination activities with greater degrees of focus. Memorial Hermann, with our contracted health plan and network partners, can offer value-based program activities designed to improve patient outcomes while reducing and/or managing costs for employers and their employees and families.
