The Memorial Hermann Accountable Care Organization (ACO) provides targeted health management services and care coordination activities with greater degrees of focus. Memorial Hermann, with our contracted health plan and network partners, can offer value-based program activities designed to improve patient outcomes while reducing and/or managing costs for employers and their employees and families.
Through a clinically integrated network of affiliated physicians and hospitals, the ACO works with Memorial Hermann contracted health plan partners to optimize outcomes that result from the benefits of data exchange and aligned collaborative care actions. Everything we do is centered around providing high-quality, cost-effective patient care delivered in a coordinated manner designed to result in optimal patient experiences.
