That is why the Diabetes Center at Memorial Hermann Texas Medical Center offers a program that begins with a private one-on-one session with a Diabetes Educator. You will receive an assessment of your diabetes needs as well as tools to help you achieve your personal health goals. After your first appointment, we offer you continued sessions based on your specific needs. You will find that our appointment times have been designed to meet a wide variety of personal schedules.

Our self-management program is recognized by the American Diabetes Association; an organization that sets the gold standard for excellent diabetes care through education, experience and credentialing. The focus of the program is based on your individual needs and personal goals and may include one or all of the following:

  • Healthy eating
  • Being active
  • Glucose monitoring
  • Medications
  • Problem solving
  • Risk reduction
  • Healthy coping

Virtual visits are available based on your insurance plan.

Translation services are available for any language. Videos and printed materials are also available for a variety of different languages.

A physician's referral is required before you can make an appointment. Please ask your physician to fax an order for Diabetes Education to (713) 704-0933. Diabetes Self-Management Education is covered by most insurance companies.

Types of Training and Therapy

Basic Diabetes Self-Management Training

  • Designed for the newly diagnosed patient and the established patient with sub-optimal control (HgbA1c³8%) whose goal is to improve glycemic control
  • Currently consists of five individual and/or group sessions over a six-month period
  • Includes Medical Nutrition Therapy

Diabetes Medical Nutrition Therapy

  • Designed for the established patient who has specific nutrition management needs only
  • The Clinical Dietitian Diabetes Educator guides the patient to establish an individualized nutrition plan and lifestyle changes to meet the patient's needs
  • The patient is then trained on the knowledge and skills necessary to promote improvement and desired outcomes
  • Number and frequency of visits is individualized and determined at time of initial assessment

Gestational Diabetes Training

  • Designed to provide training for the newly diagnosed patient on basic management of gestational diabetes and on her individual treatment plan
  • Number and frequency of visits is individualized and determined at time of initial assessment

  • Your questions and concerns
  • If you are already checking your blood sugars, please bring your blood sugar meter and logbook.
  • Your most recent lab work, if available
  • Your medication list (if applicable)
  • You are welcome to bring a friend or family member for support.
