Conroe ISD employees and their eligible dependents have access to high-quality, affordable care at Conroe ISD Employee Health and Wellness Center. We are now allowing walk-ins again. Social distancing is required.
The health and well-being of employees and their families is a priority, and we understand that busy schedules can make it difficult to schedule care. Located on the Oak Ridge Elementary School campus, the center provides a convenient and affordable option for medical care. Our services include primary care, preventive care, physical exams, vaccinations, flu shots, and treatment for minor injuries. We also provide chronic disease management for conditions such as diabetes and hypertension.
The Conroe ISD Employee Health and Wellness Center is staffed and operated by Memorial Hermann Medical Group (MHMG), a part of the Memorial Hermann Health System, one of the most trusted health systems in the nation. MHMG maintains the privacy and confidentiality of your personal health information in compliance with all state and federal laws. Personal health information will not be shared with Conroe ISD. The clinic provides regular well care and treats common illnesses and injuries like cold, flu, cough, sore throat and sinus infections. The clinic also provides chronic disease management for conditions such as diabetes and hypertension.
Yes. Dependents of all ages who are enrolled in the Conroe ISD medical plan may be treated for acute illnesses and dependents 7 years and older may be seen for wellness exams.
Walk-in patients for sick visits and acute care needs will be taken up to 45 minutes prior to closing time, or when the clinic reaches maximum capacity. Physicals, well-person, follow-up and chronic care visits should be scheduled in advance to allow sufficient time to complete the visit and promote a better patient experience.
You will enjoy convenient access to high-quality primary care services and also save time as most patients are seen within 30 minutes or less.
The cost is $10 for Conroe ISD medical plan members. For all other employees, the cost is $50.
Note: Individuals enrolled in a health savings account (HSA) are not eligible to receive services from the Center.
If you need more advanced care or testing, our experienced providers will help you arrange additional services, using specialists and other providers in your health plan. However, if you are enrolled in the Nexus ACO R Memorial Hermann medical plan, a referral can only be submitted to the insurance company if you have designated the Conroe ISD Employee Health and Wellness Center as your Primary Care Provider (the UnitedHealthcare provider ID is 00007094721002).
