Memorial Hermann Sports Medicine & Rehabilitation Kingwood West offers a wide array of services from general therapy to specialty programs, with highly skilled and certified therapists. As part of patient-centered care, therapists perform comprehensive evaluations to determine appropriate treatment to individually address a patient's specific goals.
Please note that all patients under the age of 16 are required to be accompanied by an adult, adult family member or legal guardian during their treatment time.
Patients 16 years of age or older are required to be accompanied by an adult during their initial appointment for paper work and consent purposes. After the initial visit, adult supervision may no longer be required.
If an adult is not present with the patient as listed by our age requirements, the patient may be asked to be rescheduled.
Brent Delasalas, DPT has a Bachelor of Science in kinesiology exercise science and a doctorate in physical therapy. He is board certified in physical therapy, and has advanced training in differential diagnosis. Brent specializes in rotator cuff injuries, ACL tears and ankle sprains. He also provides McKenzie Method, neurological rehabilitation and orthopedic therapy for patients ages 14 and older.
Amy Lindsey, PT, CHT has a bachelor’s in exercise science and a master’s in physical therapy. She is board certified in physical therapy and is a certified hand therapist. In practice since 2001, she specializes in hand therapy, including elbow and wrist conditions, for patients of all ages. In addition to regular clinic hours, Amy is available for early morning appointments.
Amber Shook, PT, DPT, who has earned a doctorate in physical therapy, is board certified in physical therapy. She treats sports/orthopedic injuries and post-op orthopedics, focusing on the hip, knee and foot/ankle areas. She specializes in junior high and high school concussions, ACL reconstruction, return-to-sport and patellofemoral/ITB friction syndrome. Amber has been in practice since 2018 and sees patients ages 12 and older.
Eric Stevens, PT has a master’s degree in physical therapy and has been in practice since 1999. He uses dry needling, Graston Technique/IASTM and other approaches to treat neck/shoulder, thoracic/lumbar and knee conditions. Eric treats patients ages 12 and older.
Rachael Stafford, PT, DPT has a doctorate in physical therapy and is board certified in physical therapy. She has completed advanced training in differential diagnosis, McKenzie spine techniques, manual techniques for joint and soft tissue mobilization, and is certified in dry needling and Graston/IASTM. She has been in practice since 2001, and treats injuries and conditions of all joints, the neck and back. Rachael sees patients of all ages, including as young as one month old. In addition to regular clinic hours, she is available for early morning appointments.
Cesar Fuentes, PTA has an associate’s degree in applied science/physical therapist assistant and a Bachelor of Science in kinesiology exercise science. In practice since 2016, he provides general physical therapy services to patients ages 15 – 85 in the areas of ankle-foot, knees, hip, low back, neck, and shoulder. Cesar specializes on ACL rehabilitation, hip and total knee replacement rehabilitation, as well as ankle and foot conditions. In additional to regular clinic hours, Cesar is available for early morning appointments.
