Please note that all patients under the age of 16 are required to be accompanied by an adult, adult family member or legal guardian during their treatment time.
Patients 16 years of age or older are required to be accompanied by an adult during their initial appointment for paper work and consent purposes. After the initial visit, adult supervision may no longer be required.
If an adult is not present with the patient as listed by our age requirements, the patient may be asked to be rescheduled.
David Eldridge, PT who has a Bachelor of Science in physical therapy, is certified in McKenzie Method and mechanical diagnosis and therapy. He has also completed advanced training in differential diagnosis. He does McKenzie Method, as well as joint replacement and general outpatient orthopedic therapy, concentrating on the neck/shoulder, thoracic/lumbar, foot/ankle and knee areas. In practice since 1992, he sees patients ages 12 to 90 at two locations, where he also serves as the manager. David sees patients at the Sports Medicine & Rehabilitation Alvin location on Tuesday and Thursday, and sees patients at the Sports Medicine & Rehabilitation clinic at Memorial Hermann Southeast Hospital on Monday, Wednesday and Friday.
Joby Samuel, PT, DPT, OCS, CSCS, COMT, Cert. DN has a Doctorate in Physical Therapy and is a board certified clinical specialist in orthopedics. He is a certified Strength and Conditioning specialist, a certified orthopedic manual therapist, a certified Clinical Instructor, and is also certified in Dry Needling. He is trained in Kinesio-taping, McKenzie Method for low back, Concussion therapy, Functional Movement Screens, and certified in Selective Functional Movement Assessment (SFMA). In practice since 2004, he sees patients of all ages and orthopedic conditions. His treatment approach is a combination of evidence-based practice with emphasis on manual therapy that is tailored to each individual patient.
Cassandra Woodson-Petteway, PTA who has been in practice since 1998, has an associate’s of applied science degree/physical therapist assistant and is a board-certified physical therapist assistant. She is trained in the McKenzie Method, aquatic therapy and orthopedic post-op therapy for the neck/shoulder, thoracic/lumbar and knee areas for patients ages 15 and older. Cassandra sees patients at the Sports Medicine & Rehabilitation Alvin location on Monday, Wednesday and Friday. She sees patients at the Sports Medicine & Rehabilitation Pearland West clinic on Tuesday and Thursday.
