Our rehabilitation programs take place in a supportive, patient-centered environment, where patients' goals are at the center of what we do. A customized rehabilitation treatment plan is developed for every patient with the following conditions, including, but not limited to:

• Stroke
• Non-traumatic brain injury
• Neurological disorders such as Guillain-Barré syndrome and other disorders of the nervous system
• Neuromuscular disorders such as Parkinson’s disease and multiple sclerosis
• General Rehabilitation (including debility)
• Cancer rehabilitation
• Medically complex patients with multi-system disorders
• Non-traumatic spinal cord injury
• Orthopedic conditions such as osteoarthritis, rheumatoid arthritis, fractures or complicated joint replacements
• Amputation
• Medical trauma
• Spinal surgery with neurological impairment

Interdisciplinary Rehabilitation Care

Care teams at TIRR Memorial Hermann - Southwest may include affiliated physical medicine and rehabilitation physicians, patient and family members, nurses, physical therapists, occupational therapists, speech and language pathologists, neuropsychologists, case managers, social workers, clinical dietitians, respiratory therapists and/or chaplains.

Efficient Communication

Affiliated physical medicine and rehabilitation physicians work closely with your primary care physician and other specialists to ensure continuity in your medical care, allowing them to be kept abreast of your care while you are in a rehabilitation setting.

TIRR Memorial Hermann – Southwest has also been accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF), which signals a commitment to continually improving services, encouraging feedback and serving the community.

Admission Criteria

Each patient admitted to TIRR Memorial Hermann - Southwest's inpatient rehabilitation is expected to have a diagnosis/condition which is known to benefit from comprehensive rehabilitation intervention. Functional limitations may include any of the following:

• Mobility deficits
• Activity of daily living such as grooming, dressing, or bathing
• Feeding or swallowing deficits
• Speech, language, or hearing deficits
• Cognitive, perceptual and/or sensory deficits

Patients admitted for inpatient rehabilitation services* are expected to meet the following criteria:

• Must be medically stable to allow for full participation in 3 hours of interdisciplinary rehabilitation treatments. Comprehensive therapy services may include:
• Physical therapy
• Occupational therapy
• Speech and language therapy
• Other services may include respiratory therapy and/or psychology
• Must require visits by a physician specializing in Physical Medicine and Rehabilitation and 24-hour rehabilitation nursing care
• Must demonstrate potential for functional and/or cognitive improvement in a reasonable length of time
• Must have a discharge plan that provides the necessary support based on the patient’s abilities at discharge.
• Patients with acute, infections, and/or communicable disease are not routinely admitted.
• Patients must have the physical and/or mental potential to benefit from the program.
• Patients are admitted to the program in accordance with the admission procedures. The patient's degree of impairment must be sufficient to require inpatient hospitalization.

Although the majority of our referrals come from acute-care hospitals, we also receive referral from other sources, - including, but not limited to, physician offices and skilled nursing facilities.

Funding sources may include private insurance, worker’s compensation, government programs (i.e., Medicare) and self-pay. In addition, charity care can be provided in accordance with the appropriate guidelines.
