About MyChart

MyChart offers personalized and secure online access to your medical records. It enables you to manage and receive information about your health. With MyChart, you can:

  • Schedule medical appointments.
  • View your health information, including medications, allergies, test results and more.
  • Request medication refills.
  • Access resources for trusted health information.
  • Message your care team.

No, MyChart is a free service for all patients.

You need access to a computer or mobile device connected to the internet, and an up-to-date web browser (such as Edge, Chrome, Firefox or Safari). You can also use the MyChart mobile app on a mobile device.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords. Each person has sole control over their password, and the account must be accessed using that password. On mobile, the account may also be accessed using either face ID or fingerprint login. Unlike conventional email, all MyChart messaging is done only while you are securely logged in to our website or mobile app.

You can ensure that your account stays secure – even if someone else has your username or password – by turning on two-step verification. When this feature is turned on, you must enter a code (sent to you by email or text message) to log in to MyChart in addition to using your username and password. You might receive email or text notifications that contain links to MyChart. These notifications allow you to log in to MyChart securely using a link that expires after a short time. Keeping your MyChart account secure requires keeping your phone number and email account secure. If you think that your phone number or email account may be compromised, you can change the phone number or email account on file on the "Personal Information" page in MyChart.

Your Medical Record

With MyChart, you can view most test results as soon as they become available. Know that you will likely see results before your health care provider has had a chance to review them. After your provider reviews your results, you might see additional comments and interpretation in MyChart.

You will generally receive a response within 1-3 business days. Note that MyChart messages should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

On the Personal Information page in MyChart, you can update your address, phone number, email address, preferred name and other personal details at any time, so that your clinic always has the most up-to-date information in your record.

Your MyChart information comes directly from your electronic medical record at your provider's office. Ask your provider to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record after each visit.

To obtain a copy of a deceased family member’s medical records, loved ones should contact our Release of Information department for further assistance. Due to privacy regulations and policies, requests for access to a deceased patient’s medical records are handled through the Release of Information department. For more information, visit the Medical Records webpage or call (713) 867-4335 from Monday to Friday, 8 a.m. to 4:30 p.m. CST. Technical Questions

Download the My Memorial Hermann™ app or click here to sign up. Answer a series of questions to securely create your username and password. To access your Health Records in MyChart, you will need an activation code. There are several options to get an activation code:

  • You can request one by calling your doctor’s office, calling our support line at (713) 222-CARE (2273) or emailing us at DigitalSupport@memorialhermann.org.
  • Clinic staff can provide one at your next appointment.
  • You will be able to activate MyChart after you complete registration forms before a visit.
  • You may receive a MyChart activation code in your Visit Summary paperwork after your visit.

If you're having trouble logging in, click the "Forgot Username?" or "Forgot Password?" link below the login fields for assistance. You will go through two-step verification to verify your identity, so you can recover your username or password. You can also contact our MyChart Patient Support Line at (713) 222-CARE (2273) to request assistance.

Try checking the spam or junk folders in your email. If the email with your code is not there, try clicking Resend Code. If you still didn't receive the email, it could be that your account has a different email address on file. If you have multiple email accounts, check one of the others to see if the verification code was sent there instead. If none of your accounts received the email, it could be that we don't have an email address / the correct email address on file. If this is the case, call our MyChart Patient Support Line at (713) 222-CARE (2273) for quick support.

If you don’t have an activation code, you can request one by calling your doctor’s office, emailing DigitalSupport@memorialhermann.org or calling our support line at (713) 222-CARE (2273). After we verify your information, you will receive a new code.

We aim to protect your privacy and information. If you remain idle for 10 minutes or more after you log in to MyChart, you will be automatically logged out. We recommend that you log out of MyChart if you need to leave your computer for even a short period of time.

If you've been seen at another health care organization, you might be able to view information from that medical record right in MyChart. You might have heard of this feature referred to as Happy Together. This includes various medical-related information obtained from other organizations, such as:

  • Allergies
  • Care team members
  • Health issues
  • Medications
  • Messages
  • Test results
  • Visit dates

To view this information, you must link your account. Check out the video tutorial on how to do this.

You can contact Memorial Hermann’s Patient Support at (713) 222-CARE (2273), or DigitalSupport@memorialhermann.org, for help deactivating your account. If you have connected your account’s access with multiple organizations, you will no longer be able to see the deactivated account’s data in the other organizations’ accounts. You will also no longer be able to access your deactivated account. However, your medical information and other associated data will not be deleted due to legal requirements, as well as to help optimize the care you receive from your health care providers. If any other My Memorial Hermann™ users have access to your medical information, they will continue to have access to your medical information even after your account is deactivated. You are welcome to sign up for another My Memorial Hermann account at any time in the future.

Yes. Family members and other caregivers can be granted access to another person’s MyChart account to help manage their health care. This is called Proxy Access. To be a proxy for another adult, child or adolescent, you must have your own MyChart account. If you do not have a MyChart account, call your doctor’s office, email DigitalSupport@memorialhermann.org or call (713) 222-CARE (2273).

Parents and legal guardians have full access to health records of minors under the age of 13. Patients who are 13-17 years old can have their own MyChart accounts, and their parents / legal guardians can have limited access to their health records, which may include lab results, current medications, diagnosis and history information, and provider notes.

If you are 18 years old, you can grant access to your records (for family or friends to assist with managing appointments, for example) by going to Menu>Sharing Hub>Share with a family member, friend or caretaker>Manage ongoing access
to your MyChart Account, then click “Invite Someone.”

Ages 0-12

Proxy access will automatically be granted to the birthing parent for newborn Children. To request access to a minor’s account, go to Sharing Hub>Manage friend and family access>Request access to a minor's record. Fill out the details, and we’ll respond in 1-2 business days. You may need to provide proof of identification via secure messaging in MyChart.

To be a proxy for another adult, child or adolescent, you must have an active MyChart account. If you do not have a MyChart account, call your doctor’s office, email DigitalSupport@memorialhermann.org or call (713) 222-CARE (2273). Access to a minor’s account will include all MyChart features until the child reaches the age of 12.

Ages 13-17

Patients who are 13-17 years old can have their own MyChart accounts, and their parents / legal guardians can have limited access to their health records, which may include lab results, current medications, diagnosis and history information, and provider notes. If you do not have your own MyChart account, call your doctor’s office, email DigitalSupport@memorialhermann.org or call (713) 222-CARE (2273). Parents and guardians who have access to minors’ health records will have their access revoked when the minor turns 18.

If you were an Everyday Well user, you will be able to access legacy Everyday Well medical information from May 2021 to 2024 through your MyChart account in My Memorial Hermann™. Everyday Well users seeking assistance requesting medical records created prior to May 2021 may call (713) 222-CARE (2273). If you are a patient of a Memorial Hermann independent affiliated physician, please contact your provider for instruction on accessing medical information prior to May 2024.

Some non-employed affiliated physicians use Community Connect, an extension of Memorial Hermann's Epic System, as their medical record.

Note: At Memorial Hermann Health System, we are dedicated to protecting the information of our patients. The proxy process was designed to comply with state and federal privacy laws. Parents accessing the My Memorial Hermann and Epic MyChart patient portal are able to see portions of medical record for patients ages 13-17. Parents may contact the Memorial Hermann Release of Information Department directly at (713) 222-CARE (2273) or online at Request Medical Records to request a complete copy of their child’s medical records.
